For the 2008 Cartouche we request each application contain a minimum of:

1. Candidate's Name and address information.
2. A complete biography or curriculum vitae including the candidate's
working period.
3. Photographs, preferably in color, of the candidate's outstanding
work if qualifying for a craft.
4. For persons qualifying for a non-crafts field, a narrative
describing the specific field and all remarkable achievements.
